Welcome to AgeWell Geriatrics! We are a small, home-grown geriatric primary care practice that comes to you! We are staffed by adult/geriatric trained nurse practitioners and offer everything a primary care clinic can, only we bring it to your home. We serve patients in their private homes, foster care homes, memory care facilities, independent/retirement and assisted living facilities in the Portland Metro/Salem Keizer area.

We can act as your primary healthcare provider her in the Portland Oregon Metro area and beyond. We partner with other mobile services that include imaging, phlebotomy/lab services, and mobile urgent care. We take extra time at your appointments to assure we address all of your concerns.

We can provide you with your yearly checkups, vaccines (except COVID19 vaccines and boosters, which are administered by the State), dietary education, mental health services, colon cancer screenings, and much more.

We routinely place referrals for home health and home medical equipment needs, as well as specialist referrals to other providers, such as dermatology, ophthalmology, orthopedics, Hospice, and more.
We take Medicare, Medicaid, and the following insurance plans: United Healthcare, Regence, Moda, Aetna, Atrio, Healthnet, Tricare, Trillium, Wellcare, Providence Health Plans, and private pay.
Currently do not accept: Humana, Kaiser, VA benefits, Pacificsource
